So you have dental insurance with an Orthodontic benefit?
We are more than happy to file your insurance claim and accept your insurance benefit, saving you time and reducing personal costs. Many dental insurance policies have an orthodontic lifetime maximum that is separate from standard dental insurance. We would greatly appreciate you investigate your policy and be keenly aware of any limitations of the benefit provided.
Please remember that orthodontic insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
The fees we charge for services rendered to those who are insured are our usual and customary fees charged to all patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies pay claims promptly and others delay payments many months.
Our customary procedure in handling our patients’ accounts is the same for patients with or without insurance coverage. An estimate of the fee is given. We then request that an initial fee be paid at the start of treatment. The remainder can be paid in monthly installments.

PAYMENT OPTIONS
The fees cover the cost for appliances and services performed in this office during active treatment followed by an additional one year of retention treatment. Additional charges will be made for appliances lost or broken due to negligence. The fee does not include any general dentistry, X-rays or extractions needed during or related to treatment performed outside this office.

FULL PAYMENT AT START OF TREATMENT
There is a discount from the total treatment fee if payment is received in full at the beginning of treatment.
INTEREST FREE IN-HOUSE FINANCING PLAN
We offer interest free in-house financing. An initial down payment of 20% - 25% is made when treatment starts. The remaining balance is paid in monthly installments autodrafted from your checking, savings, credit card or flexible spending account.
